The National Testing Agency (NTA) has prescribed a process for candidates who require a duplicate scorecard because the original has been lost, stolen, damaged or destroyed. The facility is applicable to candidates of various examinations conducted by NTA, including NEET UG, JEE Main, UGC NET, CSIR NET and other eligible exams.
Before applying for a duplicate scorecard, candidates should first check whether their original scorecard is available through DigiLocker. If it is available there, candidates can download the digital copy without submitting a separate duplicate-scorecard request.
Candidates can follow the process below to request a duplicate copy of their NTA scorecard.
The first step is to access the candidate's DigiLocker account and search for the relevant NTA scorecard.
If the scorecard is available, download the digital version directly.
In such a case, there is no need to submit a separate duplicate-scorecard request.
If the scorecard cannot be found on DigiLocker, candidates need to download and fill out Annexure-I mentioned in the NTA notification dated September 17, 2026.
Applicants should provide all the information requested in the prescribed form and ensure that the entries are accurate.
Candidates must attach the required documents and supporting records along with the completed application.
Before sending the request, applicants should check that all documents are complete, properly arranged and meet the requirements mentioned by NTA.

Candidates requesting a duplicate NTA scorecard should:
Check DigiLocker before submitting an application.
Use the prescribed Annexure-I form if the scorecard is unavailable digitally.
