The National Testing Agency (NTA) has a provision for candidates who have lost, damaged, misplaced or otherwise become unable to use their original scorecard and need a duplicate copy. The facility applies to scorecards issued for JEE Main, NEET UG, CUET PG and other examinations conducted by NTA, subject to the applicable NTA procedure.
Before requesting a duplicate scorecard, candidates should first check whether their scorecard is available through DigiLocker.
Candidates may request a duplicate scorecard when the original document has been:
Lost or stolen
Damaged or mutilated
Destroyed because of fire, flood or another circumstance beyond the candidate's control
Candidates who cannot obtain their scorecard through DigiLocker can follow the duplicate scorecard procedure prescribed by NTA.
The application form is available in Annexure I of the relevant NTA notification. Candidates need to complete the form and submit it along with the required supporting documents according to the instructions provided by NTA.
NTA also permits candidates to send scanned copies of the required documents in a single PDF to [email protected]. Candidates choosing the email option are also required to send the documents physically through Registered Post or Speed Post, as specified in the NTA procedure.
The applicable fee depends on the number of years that have passed since the declaration of the examination result:

Candidates should verify the applicable fee and payment instructions in the latest NTA notification before submitting their request.
Applicants may need to submit the following documents along with their duplicate scorecard request:
Completed and signed duplicate scorecard application
Affidavit in the prescribed format
Copy of the police report, where applicable in cases of loss or theft
Two recent colour passport-size photographs
Copy of the admit card or registration slip, if available
Proof of payment of the prescribed duplicate scorecard fee
Self-attested government-issued photo identity proof
Indemnity bond and undertaking in the prescribed format
Candidates should carefully read the latest NTA notification and its annexures to confirm the exact documents and format required for their application.
Candidates should first log in to DigiLocker and check whether their NTA scorecard is available there.
If the scorecard can be retrieved through DigiLocker, there is generally no need to submit a separate request for a duplicate physical scorecard.
Candidates who are unable to access their scorecard through DigiLocker can proceed with the duplicate scorecard application process prescribed by NTA and submit the required documents within the applicable procedure.
