Age-related macular degeneration (AMD) is the leading cause of irreversible central vision loss in adults over 50 in high-income countries, affecting ~200 million people worldwide. The disease is driven by the convergence of:
- Drusen formation and Bruch's membrane thickening (lipid deposition, RPE failure)
- Complement system dysregulation (CFH Y402H, ARMS2 A69S genetic variants)
- VEGF-driven choroidal neovascularization (CNV) — wet/neovascular AMD
- Geographic atrophy (GA) — progressive RPE and photoreceptor death (dry/atrophic AMD)
This QSP model integrates all four major biological axes into a 20-ODE compartmental system capable of simulating anti-VEGF pharmacokinetics, VEGF/Ang-2 pathway suppression, complement activation, RPE cell dynamics, and clinically observable endpoints (BCVA, OCT-CST, CNV area, GA area).

Anti-VEGF binding affinity drives sustained suppression: Aflibercept (Kd ~0.5 pM) achieves stronger and more durable VEGF capture than ranibizumab (Kd ~40 pM) per molecule, but the larger dose of brolucizumab/faricimab compensates with high molar drug concentration.
-
Dual VEGF/Ang-2 blockade (faricimab): Ang-2 destabilizes vessels and amplifies VEGF-driven leakage; simultaneous neutralization synergistically reduces fluid and extends treatment intervals to q16w in ~45% of patients (TENAYA/LUCERNE).
-
Complement amplification of GA: Local C3/MAC deposition accelerates RPE atrophy, creating a vicious cycle (RPE death → impaired complement regulation → more MAC → more RPE death). Pegcetacoplan (C3 inhibitor) demonstrated ~22% slowing of GA in OAKS/DERBY trials.
-
Lipofuscin A2E as drusen driver: A2E accumulation in RPE lysosomes impairs phagocytosis → drusen growth → complement deposition. This feed-forward loop is the central driver of disease progression from intermediate to late AMD.
-
BCVA as composite endpoint: The model captures how visual acuity reflects contributions of CNV area (hemorrhage/fluid), GA area (photoreceptor loss), CST elevation (fluid distortion), and photoreceptor survival — providing mechanistic interpretation of clinical trial endpoints.
