One small update to the meeting link
…re WG Per the 2026-09-21 group meeting (discussion modelcontextprotocol#3375): rename the group to Progressive Disclosure, convert the charter to a Working Group, add Kurtis Van Gent as Core Maintainer, point links at the renamed progressive-disclosure-wg repo and Discord channel, and set a weekly meeting cadence.
