Fixes #475.\n\nThe README documented a /create-plugin command and a Typical flow starting with it, but the plugin ships no commands directory and the manifest declares only skills, rules, and agents. The actual scaffolding entry point is the create-plugin-scaffold skill.\n\nThis drops the fictional Commands section and points step 1 at create-plugin-scaffold. Verified against current main: no commands/ directory, manifest has no commands key.

Overview
Aligns create-plugin README with what the plugin actually ships: it removes the Commands table that referenced a non-existent /create-plugin command and updates the Typical flow so step 1 directs users to the create-plugin-scaffold skill instead of that slash command.
