fix: add missing columns to stateful shell ack rows - #4921

The stateful shell session chain(a={}) row only included CommandId and
Timestamp, missing Command, Stdout, Stderr, StdoutUpload, and StderrUpload
columns that the non-stateful path already provides.
When the transcript notebook evaluates 'SELECT Command + Stdout AS Line
FROM source()', rows without Stdout produce 'Symbol Stdout not found'
errors. Add the missing columns to match the non-stateful chain(a={})
pattern for all three shell artifacts.

Fixes #4914. Stateful shell session path lacked Stdout/Stderr columns, causing transcript notebook to fail with Symbol not found. Added placeholder columns to all 3 shell artifacts. Signed-off-by: Yuchen Fan <functionhx@gmail.com>
