WHO COULD FORGET THAT MOUTH -- LIKE THE FRONT grille of a 50's roadster. And those headlight eyes. It must be Dolly Levi a k a Carol Channing: the two have never been far apart since the actress opened in the original "Hello, Dolly!" on Broadway in 1964. The Yonkers matchmaker is setting up shop at the Lunt-Fontanne Theater, where previews of the Jerry Herman musical begin on Wednesday. It opens on Oct. 19. Al Hirschfeld has chronicled a chorus of Carol Channings over the years, including that first David Merrick production in which the performer was followed by, among others, Pearl Bailey and Ethel Merman.
