add CONTRIBUTING.md #51

tl;dr

if you see something that needs doing, it's your job

we could just copy OPEN open source, or previous contributing guidelines i've written include holodex and value flows in case we want to re-use any content there.

plus we may want to include any recommendations on code style, README style, etc to provide somewhat consistent quality across ecosystem modules.

any thoughts @dominictarr?
