Gastroparesis (위마비) — QSP Model

Mechanistic Map

Disease Overview

Gastroparesis (위마비, delayed gastric emptying) is a chronic gastric motility disorder defined by objectively delayed gastric emptying in the absence of mechanical obstruction. The hallmark is reduced gastric emptying rate resulting in symptoms of nausea, vomiting, early satiety, bloating, and postprandial fullness.

Prevalence: ~5 million US adults; diabetic (~29% of T1DM, ~1% of T2DM), idiopathic (~36%), post-surgical (~13%)

Diagnosis: Gastric emptying scintigraphy (GES) — >10% retention at 4 hours is the gold standard

Pathophysiology Summary

Hyperglycemia / DM
    ↓
Oxidative stress (ROS ↑, nNOS ↓)
    ↓
ICC loss (c-Kit+ pacemaker cells ↓) + nNOS neuron depletion
    ↓
Impaired gastric slow waves → ↓ antral contractions
    ↓
Pyloric hypertonicity (unchecked NO-mediated relaxation)
    ↓
Delayed gastric emptying (GER ↓, 4h retention ↑)
    ↓
GCSI symptoms: nausea, vomiting, early satiety, bloating

Three Core Defects:

  1. ICC Loss — Interstitial cells of Cajal (pacemaker cells) depleted by 40–60% in DM-gastroparesis; drives slow-wave dysrhythmia
  2. nNOS Neuron Depletion — Neuronal nitric oxide synthase-expressing neurons absent in ~85% of gastroparesis biopsies; loss of pyloric relaxation
  3. Vagal Neuropathy — Impairs central regulation of antral contractility and gastric accommodation

Drug Targets & Treatment Classes

Drug Class Main Target Prokinetic Mechanism
Metoclopramide D2/5HT4 D2 receptor (GI+CNS) D2 block → ↑ACh; partial 5HT4
Domperidone D2 antagonist Peripheral D2 No BBB penetration (safer EPS)
Erythromycin Motilin agonist Motilin receptor Phase III MMC induction
Prucalopride 5-HT4 agonist 5-HT4 receptor Full agonist → ↑ACh, antral drive
Relamorelin Ghrelin analogue GHSR Antral contractility ↑
Ondansetron 5-HT3 antagonist CTZ 5-HT3 Antiemetic (not prokinetic)
