One assignment, one coherent fix

Many scheduled and PR workflows finish with missing_terminal_safe_output, most classified as prompt_exhaustion, so no outcome is recorded.

Implementation scope

Add an early context-budget guard and a final-turn reminder in the engine prompt wrapper that forces noop or report_incomplete before exhaustion, and include the classification and last tool in the failure issue.

Done when

  • A regression test shows an exhausted run still emits report_incomplete.
  • Failure issues for exhausted runs name the budget consumed and the last tool call.

Why now

At least a dozen recent failures across unrelated workflows share this signature, so one wrapper-level change has fleet-wide impact. Some runs are classified unknown, which leaves residual uncertainty.
