daily-go-test-parallelizer.md configures engine: { id: codex, model-provider: github } with model: copilot/gpt-5.6-terra (lines 14–17), which routes the codex engine through GitHub's Copilot-compatibility adapter. That adapter only supports the apply_patch custom tool call, but the codex harness emits an exec tool call, so every run that reaches a tool call fails immediately with invalid_request_error / unsupported_custom_tool:

"Unsupported Responses custom tool call 'exec'. Only 'apply_patch' is supported through the Copilot compatibility adapter."

This was first auto-filed as #65950 (2026-10-05), auto-closed/expired on 2026-10-06 without a fix landing, and has since recurred continuously — at least 8 separate auto-filed failure/incomplete-result issues for this workflow on 2026-10-07 alone (#66392, #66394, #66411, #66439, #66440, #66481, #66513, #66514, #66570, #66604, #66605, #66628, #66629, #66653, #66683, #66697), per the Agent Job Health Monitor's 2026-10-07 report (#66705): "Daily Go Test Parallelizer: 3 runs, 2 failures, 66% failure rate" and recommendation #4 to re-file since "the expiry mechanism silently dropped a real, repeating bug." This is currently the single highest-frequency failure signature in the fleet.
