When a name is written in one case, a particle that is also a credential (vd, mc) gets pulled out of the middle of a family name or a given part. That leaves a family name that isn't in source order. #554 settled that these words count as credentials after a comma. These shapes are different: one is before the comma, the other is in the given part's trailing slot. The writing that #554 treated as decisive (uniform case) is exactly where they break.
One name word before a family comma:VD stays behind in the family, cut off from the word it heads. That is neither reading: as a particle the family is SMITH VD MA (2.3.0's reading), and as credentials it is family SMITH, suffix VD MA, the way SMITH, PHD MA reads.
