docs: scrub stale [fastapi] extra references
The [fastapi] extra was removed in v0.6.0 (commit aa9a009 — it was a non-functional shim that never declared optional deps) but README.md and examples/README.md still told users to pip install pluginforge[fastapi]. Both Bibliogon and AdaptivLearner reported tripping over this during adoption. README.md now installs FastAPI as a normal dep alongside pluginforge, with a one-line note explaining the removed extra. examples/simple_app install command updated to match. CHANGELOG.md line 165 (the v0.6.0 historical entry documenting the removal) is intentionally left untouched. pyproject.toml uses readme = "README.md" so the PyPI long_description inherits this scrub at the next build. Doc-only; no code paths touched.
