Keep changelog paragraphs on single lines (#20778)
The 0.12 release notes hard-wrap paragraphs and separate list entries
from their pull request references, which breaks GitHub rendering. Keep
each changelog paragraph and its references on one line, and require
future generated release notes to follow the same convention.
